Thank you for considering SNFC to be involved in supporting your fundraising initiative. We are proud to support our community, and are dedicated to helping our members and communities live a healthier life, achieve results and reach their potential.

Complete the below form in order to request a membership donation to support your charitable event or fundraising initiative. Please note that your request must be submitted at least 3 weeks in advance of your deadline in order to be considered.

All requests must be received a minimum of 3 weeks in advance of your event via the online application form. Membership donations are limited.

Organizations that are selected will be awarded a membership certificate valid at select or all locations. The membership must be redeemed at a SNFC location by the date stated on the Gift Certificate.

You will be contacted if your submission is accepted to receive a donation. Due to the volume of requests received, only those that are granted membership donations will be contacted.

All membership donations must be accepted as awarded. No cash value.
